Investigation after prisoner dies at women's jail

An investigation has begun after the death of a prisoner at a women's jail.
Sarah Boyle, 35, died on 20 July after being taken from HMP Styal in Cheshire to Wythenshawe Hospital.
HM Prison Service confirmed her death would be looked into by the Prisons and Probation Ombudsman (PPO).
Ombudsman Adrian Usher said a "live investigation" was under way, with a final report due following an inquest into Ms Boyle's death.
Speaking on behalf of the independent organisation, Mr Usher offered "sincerest condolences" to Ms Boyle's family and friends.
It comes after former inmates of HMP Styal described the prison, where a number of prisoners have died, including by suicide, since 2007, as "hell on earth".
