A foundation which helps vulnerable patients when they return home from hospital has started supporting another NHS Trust in Surrey.
The Juliet O Foundation, set up by rapper and Top Boy actor Dave and his mother, former health and social care worker Juliet Omoregie, is now supporting the Ashford & St Peter's NHS Trust, as well as the St George's, Epsom and St Helier Hospitals and Health Group.
The trust said the foundation will donate a collection of discharge bags for staff to share with some of the most vulnerable patients as they leave hospital.
The bags include soap, toothpaste, tea and coffee, and non-slip socks to prevent falls.
A spokesperson for the trust said: "We're so grateful to Dave, Juliet, and the whole Juliet O Foundation team for their compassion and commitment to helping those most in need.
"Together, we're making sure every patient leaves hospital with dignity, warmth, and hope."
Ms Omoregie worked for the NHS for more than 35 years and launched the charity when she retired two years ago.
