A volunteer member of staff was seriously injured after being hit by a train wagon, the Rail Accident Investigation Branch (RAIB) said.
The Gartell Light Railway volunteer was injured near to the Common Lane level crossing in Yenston, Somerset at 09:35 BST on 17 April.
The railway was not open to the public when the incident occurred.
The RAIB said the train was moving at low speed when the person attempted to get off the wagon and was struck by it.
"We have undertaken a preliminary examination into the circumstances surrounding this accident," the RAIB added.
"Having assessed the evidence which has been gathered to date, we have decided to publish a safety digest."
The Gartell Light Railway was approached for comment and the investigation continues.
