Jeremy Hunt: Drug errors causing four to five deaths a day

Jeremy Hunt has announced plans to "decriminalise dispensing errors" in an effort to encourage pharmacists to come clean about errors they may have made.

Mistakes made in the prescription, dispensing and monitoring of drugs are causing four to five deaths a day in England, the health secretary said.
