United Lincolnshire Hospitals NHS Trust was given a "requires improvement" rating after Care Quality Commission (CQC) inspectors visited Boston and Lincoln hospitals.
The watchdog said there were "significant concerns" about urgent and emergency care at both hospitals.

The new head of tourism organisation Visit Lincolnshire says he's working with councils to look at reducing car parking charges in the centre of Lincoln.
Mike Timson has taken over the top job at the organisation which helps drive up visitor numbers and investment in the city and the surrounding area.
He says he'd like to see a park and ride service introduced in the south of Lincoln.
It should be a dry end to the day in Lincolnshire.

Motorists in Boston are hoping for a less stressful journey home tonight after the main route through the town was shut this morning because of a burst water main.
The closure of John Adams Way caused long delays.
Three of the four lanes reopened during the morning.
Two schools in the town were closed today because of the burst.
A date is still to be set for when the remains of Lincolnshire-born explorer Matthew Flinders will return to the county.
The remains of Captain Flinders were discovered in January by archaeologists working on the HS2 project.
The explorer is the first know person to have navigated around the entire coast of Australia and is also credited with giving the country its name.
His final resting place will be at the Church of St Mary and the Holy Rood in Donington, near Spalding, where he was baptised, and where many members of his family are buried.
A date for his reburial will be announced at a later time.

The Red Arrows arrived back in Lincolnshire this lunchtime from their tour of North America and Canada.
The aerial display team landed back at their base at RAF Scampton after an 11-week tour, which included displays in New York, San Francisco, Vancouver and Washington D.C.
Families and friends were waiting to greet the crew when the touched down.
It should be a dry afternoon in Lincolnshire, though there's the slight chance of a shower in places.

More special constables are needed to join Humberside Police after the force said it had seen a drop in numbers over the last three years.
Special constables are unpaid volunteers who have full police powers, uniforms and equipment.
Some of these trained volunteers have gone on to become full-time regular officers.
Assistant Chief Constable Paul Anderson said: "We have been actively recruiting over the past two years and I do not think it will be any surprise that of the pool of people that have joined the regulars, a large number have come from the specials."
Police say they are "very concerned" for the welfare of 14-year-old girl reported missing from the Spalding area.
Summer Grimes was last seen at about 19:30 on Tuesday.
Lincolnshire Police say they believe she may be in the Sutton Bridge and King’s Lynn area and have appealed for anyone who sees her to come forward.
She is described as white, with shoulder-length auburn hair, and about 5ft 2ins tall.
She was last seen wearing black ripped jeans, a red top, and carrying a grey shoulder bag.

United Lincolnshire Hospitals NHS Trust (ULHT) was given a "requires improvement" rating after visits to Boston and Lincoln hospitals.
The trust said it was disappointed and was taking action.
Two people have been injured in a crash involving three vehicles on the A52 near Boston this morning.
The collision happened at about 05:00 near to the junction with Blackjack Road in Drayton.
Lincolnshire Police said a woman had been seriously injured in the crash, though her injuries are not believed to be life-threatening, and a man had suffered minor injuries.
Boston Grammar School and Boston Pioneers have said they will be closed today as a result of a burst water pipe in town.
John Adams Way has been closed since about 05:00 as a result of the leak.

A major route through Boston is closed this morning because of a burst water main.
John Adams Way and Haven Bridge have been closed between South Square and Liquorpond Street since about 05:00.
Anglian Water says it is working to resolve the issue but that it is a "complex job that is taking longer than we had originally hoped".
It said it hoped the work would be completed by 17:00.

The remains of an explorer credited with giving Australia its name will be reinterred in Lincolnshire after his skeleton was discovered by archaeologists working on the HS2 project.
Captain Matthew Flinders, from Donington, became the first known person to navigate around the entire coast of Australia, confirming it as a continent.
His remains were among 61,000 skeletons found in St James's Gardens, London, in January.
HS2 bosses have now said his remains will be buried at St. Mary and The Holy Rood following a request from his family and members of the local community.
Helen Wass, HS2 Head of Heritage said: “It is fitting that the last voyage of Captain Matthew Flinders will be back to the village of Donington where he grew up and we are pleased to be playing our part in his last journey."
