The fan fell ill in the Newmarket Road End of the ground a few minutes after the start of the second half of the League One fixture.
The nature of his illness has not been disclosed.
Almost 30 minutes of stoppage time was added when the match resumed. It ended in a 0-0 draw.
Cambridge United chief executive, Ian Mather, told the BBC: "Fans at the Newmarket Road End made a signal to our goalkeeper, Dimi Mitov, to say something was wrong in the crowd.
"Dimi waved frantically to stop the game - it was pretty obvious there was a problem.
"It was worrying at the time - but this is part and parcel of running a professional football club when a match is on. Staff were there in seconds."
Mr Mather said it was humbling to see fans - including 1,500 Portsmouth away supporters - applaud the medical teams when play was stopped.
