QPR survived a scare to beat League Two Leyton Orient on penalties and reach the second round of the Carabao Cup after a 1-1 draw.
Championship side Rangers took the lead in the 16th minute when Rob Dickie got on the end of Faysal Bettache's corner and powered in a header from the edge of the six-yard box.
But Orient rallied in the second half at the Breyer Group Stadium, and Aaron Drinan equalised with 16 minutes left after Omar Beckles had flicked on Tom James' long throw.
And the east Londoners could have won it, as R's keeper Jordan Archer made good saves to repel a volley from Ruel Sotiriou and tip a drive from Beckles onto the crossbar.
But Sotiriou fired his spot-kick against the bar with the score at 2-1 to QPR in the shootout, allowing Albert Adomah to seal a 5-3 win on penalties.
QPR, who made seven changes from their opening-day draw with Millwall, put together some slick passing moves in the first half as Adomah fired over when well placed inside the box and Lyndon Dykes sent a volley over after a fine counter-attack.
Orient had twice come close to equalising at the end of the first half as Drinan and Dan Happe sent efforts wide, and the east London club put in a spirited showing after the break and dominated the closing stages.
However, O's keeper Lawrence Vigouroux could not get near any of the Rangers penalties and Adomah celebrated netting the decisive spot-kick for the 1967 League Cup winners by embracing travelling fans in the stands.
