Sheffield Wednesday needed a stoppage time equaliser and penalties to avoid an EFL Cup upset by Stockport County at Hillsborough.
Paddy Madden scored a superb volley to give the League Two side an early lead and for much of the night it appeared enough for victory.
But in the final minute of seven added on, Tyreeq Bakinson scored the scrappiest of equalisers after a scramble in the six-yard box.
Wednesday goalkeeper Devis Vasquez then saved two penalties in the shoot out before Lee Gregory scored the winner for the hosts.
It was cruel on Stockport, who were on course for a shock after Madden's stunning goal.
He met an outswinging cross from the right-hand touchline near the penalty spot and made the sweetest contact to the power the ball high into the net.
Wednesday's goal came from the opposite end of the spectrum as a high cross from the left spent an age in the six-yard box before Bakinson eventually poked home.
It spared the blushes of manager Xisco Muñoz who made seven changes from their defeat by Southampton on Friday night.
Until the goal, Anthony Musaba had Wednesday's best chance of the night as he rounded the Stockport goalkeeper only for Ethan Pye to make a last-ditch block for the visitors.
Ashley Fletcher shot narrowly past the post in the second half and Barry Bannan flashed a half-volley wide in the closing stages.
