Care Minister : Gaps in care 'nightmare for NHS'

There is a growing number of people in the UK of people who are not in hospital but need care at home.

The government has said that it has a plan for England to deal with this growing expense.

Within five years all areas of the country will have to link up NHS and adult social care services to provide a seamless service to patients, it says.

Speaking to the Today programme's Sarah Montague, Care Minister Norman Lamb explained that too often elderly and disabled people fall through the cracks in the current system.
