NHS complaints investigations inadequate, says review

The NHS in England has been severely criticised for the way it investigates allegations of avoidable harm and deaths.

A review by the Parliamentary and Health Service Ombudsman found that four out of every ten internal investigations it re-examined were inadequate and some were described as appalling.
