Poor end-of-life care 'lets down patients'

Thousands of dying patients are being let down by poor end-of-life care, according to the organisation that investigates complaints about the NHS in England.

The Parliamentary and Health Service Ombudsman says too many people are dying without dignity and spending their last days in unnecessary pain.

The Dying Without Dignity report said it had found too many instances of poor communication - along with poor pain management - and inadequate out-of-hours services.
