Teen's diary reveals mental health ordeal

Sarah Green was a 17-year old girl who died as a result of self-harming while an inpatient in a mental health unit.

The BBC's Panorama has been given access to Sarah's diary, which shows how she was failed by the child and adolescent mental health system in England.
