Mother: 'This should never have happened'

Ivy Morris was born in extremely poor conditions and died in May 2016 from complications associated with her birth.

BBC News has learned there have been at least seven deaths in less than two years at the Shrewsbury and Telford Hospital NHS Trust that were later deemed to have been avoidable.
