Sara's Story: The diary of a teenager who died in the mental health system

Sara Green, 17, died as a result of self-harming as an in-patient in a mental health unit. BBC Panorama was given access to Sara's diary that she kept, showing how she was failed by the child and adolescent mental health system in England.
