Coroner blames "cascade of failures" for patient's death

A coroner has ruled that the death of a hospital patient who was so thirsty he called police for help, was partly due to neglect by medical staff.

Twenty two-year-old Kane Gorny died at St George's Hospital in south London where he was recovering from surgery.
