Hamzah Khan death 'could not have been predicted'

A serious case review has found that four-year-old Hamzah Khan, who was starved to death by his mother, was let down by the agencies that were supposed to protect him.

Professor Nick Frost, chairman of Bradford Safeguarding Children Board, said that although the death could not have been predicted, there was no multi-agency assessment, Hamzah's siblings were not listened to and his mother's explanations should have been followed up more closely.
