Hospital failure death woman 'missed terribly'

The family of a woman who died hours after being told she was ready to leave hospital have spoken about their loss.

Sian Hollands died after doctors failed to examine, diagnose and treat her for a pulmonary embolism, Kent Coroner Roger Hatch said.

Her sister Ebony Hollands said Sian was "one in a million".
