Inquest begins into Sydney cafe siege
An inquest into the Sydney cafe siege has begun trying to establish how the hostages died and whether their deaths could have been avoided.
Hostage Katrina Dawson was killed by fragments from a police bullet or bullets and another hostage, Tori Johnson, was killed by a bullet from gunman Man Haron Monis, the inquest has heard.
