Who decides what culture is?

Only 8% of the population actively consumes culture in the UK, according to Enriching Britain, external, a study by Warwick University.
But perhaps it depends on what you consider to be culture.
For example - why does opera count when cinema does not? And are art institutions, and the people who run them, pandering to their mates?
