    Almost 700 convicted sex offenders have been removed from the register in the past four years, the BBC has learned.

    Since 2012 at least 170 rapists and 157 child abusers, on application, were told they no longer had to register with the police.

    They include people convicted of raping boys and girls, incest, and taking indecent images of children.

    In the West Midlands 13 applications - allowed under a 2010 Supreme Court ruling - were successful, a Freedom of Information request revealed. It means the force is among those to have approved the fewest applications.

    More than 30 people, including 15 children, were evacuated from a block of flats in Handsworth, after a fire in the basement. 

    Six engines were called to Lyndon Close on Sunday afternoon.

  5. Rebecca Kandare serious case review: Her death was 'preventable'published at 10:41 Greenwich Mean Time 21 March 2016

    The review concludes: "There were specific opportunities that were missed, where a different response by professionals should have led to a reassessment of Rebecca’s needs.

    "In the light of the post mortem findings of chronic malnourishment, pneumonia and severe rickets being the cause of Rebecca’s death it was agreed that, if there had been contacts with health visiting and GP services, the signs and symptoms in Rebecca would have been noticed. 

    "This would have led to appropriate services being provided by all agencies, which would have prevented Rebecca’s death."

  6. Birmingham City Council 'not making progress quickly enough' after critical reportpublished at 10:18 Greenwich Mean Time 21 March 2016

    An independent panel set up to help Birmingham City Council improve how it runs itself - its governance - says some good foundations for improvement have been made, but the changes have not yet had a big enough impact. 

    The panel was established in December 2014 after criticism of the authority in a report by the former head of the civil service Sir Bob Kerslake.

    That report made 11 recommendations for the council, including turning around its failing children's services, which had been in special measures since 2009. 

    In a statement the council said it accepted there was still more work to be done and at a faster pace and leaders had no intention of "taking our foot off the gas".

  9. Rebecca Kandare serious case review: 'Opportunities were missed to intervene'published at 09:23 Greenwich Mean Time 21 March 2016

    The serious case review into Rebecca Kandare's death has made a number of recommendations, including ensuring professionals have a better understanding of how religious beliefs can impact upon a child’s health and improving the way agencies work with families who are reluctant to engage with services.

    “Opportunities were missed to intervene – and had they been taken it might well have made a difference; that is a bitter disappointment to all concerned,” said Alan Coe, chair of the Wolverhampton Safeguarding Children Board which published the review.

    He however stressed: “Rebecca’s parents were responsible for her death. They did everything they could to disguise their neglect of their daughter while superficially complying with health professionals in particular.”

    A serious case review into the death of a malnourished baby girl who had severe pneumonia and rickets has found she was not registered with a GP and last saw a health visitor when she was just 22 days old.

    Rebecca Kandare from Wolverhampton was aged eight months when she died two years ago.  

    Her parents Brian and Precious Kandare are serving prison sentences for manslaughter

    They had attended a church which preferred members to use faith healers instead of doctors. 

  11. Ladywood shooting: A suspect is bailed; family reveals Kenichi Phillips was expectant dadpublished at 08:37 Greenwich Mean Time 21 March 2016

    A 22-year-old man arrested in connection with the death of a Birmingham teenager has been released on police bail while investigations continue.

    Kenichi Phillips, 18, was killed as he sat in a Seat Leon on St Marks Crescent shortly before 19:00 GMT on 17 March. He was an expectant father, his family said.

    His father, who did not want to be named, said Kenichi "was loved and will not get the chance to see his son born".
