    Shrewsbury and Telford Hospital NHS Trust failed to implement previous recommendations from investigations, Donna Ockenden's report has highlighted.

    If it had, some of the cases heard by the inquiry "might not have occurred" it added.

    The 27 recommendations for the hospital trust include twice daily consultant-led ward rounds, drug use following national guidelines and clearer responsibilities for the care of high-risk women.

    There are also seven recommendations for every maternity unit in England including staff training and working together, listening to women and families and a wider team of people getting involved in investigating incidents.

    A review into poor maternity care has found that some mothers were blamed for their babies' deaths, women were denied appropriate care and deaths were often not investigated properly.

    The inquiry, into services at the Shrewsbury and Telford NHS trust, also found a higher than average number of women died in labour or shortly afterwards and a drug to induce labour was repeatedly misused.

    The report, which reviewed 250 cases, lists 27 actions the trust must immediately carry out.

    A taxi driver and two police officers have been hurt in a crash in Birmingham.

    The officers were driving a police vehicle on blue lights to reports of a car theft when they were in collision with a taxi at the junction of College Road and Sunnybank Avenue last night, West Midlands Police said.

    The female PC behind the wheel was unconscious for a few minutes and, when she recovered, was found to have suffered whiplash and bruising.

    The other officer, a male PC, and the taxi driver also both suffered whiplash and all three were taken to hospital but have since been discharged.

    The force said the crash had now been passed to its professional standards team for investigation.

    A solar farm could be used to power New Cross Hospital in Wolverhampton.

    The City of Wolverhampton Council has submitted a planning application to redevelop land at the former Bowman's Harbour site, which has become blighted by anti-social behaviour and fly-tipping.

  6. Firefighters stop industrial estate fire spreadingpublished at 09:29 Greenwich Mean Time 10 December 2020

    Fifty firefighters have been battling the blaze at an industrial estate in Birmingham this morning.

    They were called to Tyseley Industrial Estate just before 04:45 and found a "significant blaze".

    The fire service said they managed to stop it spreading to neighbouring buildings, external.

    Seeleys Road at the site has been reopened to businesses not directly affected by the fire but people are being told to be cautious if they are heading into the estate.

    Crews are likely to remain on scene throughout the day and investigations will start to establish the cause of the fire.

    The ambulance service said no-one was hurt in the fire.

    Birmingham City earned back-to-back Championship wins for only the second time this season as they held on with 10 men to beat Reading.

    Blues saw out the final 19 minutes a man short after captain Harlee Dean was sent off, but keeper Neil Etheridge dived to his right to save the spot-kick and earn Blues victory.

    Jon Toral scored both goals and Birmingham's head coach Aitor Karanka said if he keeps it up, he'll be good for the team.

    "He has been waiting for his chance. He started the season well, then he got injured and he's now desperate to show what he can do."

    A fire has broken out at a unit on an industrial estate in Birmingham.

    The "significant" blaze has closed a road around Tyseley Industrial Estate.

    Firefighters were called shortly before 04:45 to the Seeleys Road site.

    "This is a serious fire and crews are working hard to contain this incident and prevent further spread to other industrial units on the estate," said West Midlands Fire service.

    "Police and ambulance are supporting us on scene and the incident is likely to be protracted."

    The Coventry City manager, Mark Robins, said the demands of Championship are starting to show on his players.

    His side drew 0-0 against Luton at St Andrews last night, meaning they stay 18th in the table and Robins said "the energy levels weren't there".

    A temporary camera is going to be set up at a busy Coventry junction after complaints about speeding cars.

    People living near the junction of The Chesils and Knoll Drive in Styvechale said they had witnessed a number of collisions.

    The council has promised additional crossroad warning signs.

    But the residents, backed by a 700-name petition, have asked for more and the council said the camera would be set up for two or three weeks to monitor the problem.

    An new school is being planned, to replace the existing Peterchurch primary in Herefordshire.

    Herefordshire cabinet members voted to set aside a budget of £300,000 for the design and want the building to be carbon neutral.

    Councillors have also asked for the buildings to be well insulated, with efficient heat recovery, to reduce running costs.

    The next step will be a feasibility study.
