Nurse Gabi Turner captured the damage to the Gara Bridge near Loddiswell in Devon after the truck hit it on Tuesday morning.
"He couldn't get through because the truck was so wide, so he stuck his foot down and had another go at it," she said.
"I was horribly disappointed because it's a very old and beautiful bridge and such a lot of damage has been done to it."
She reported the incident to the police who confirmed they are investigating.
The bridge has been closed and is due to be repaired by Devon County Council from 28 May to 4 June.

Plans to improve one of the main roads into Exeter are set to get the green light.
The £2.2m junction improvements at the Moor Lane roundabout are set to be approved by Devon County Council’s Cabinet next Wednesday., external
There are long traffic queues at peak times at the roundabout, often going all the way back to junction 29 on the M5.

A postponed election for a North Devon Council seat will take place on 13 June.
Voters were due to go to the polls last Thursday in the Chittlehampton ward for the district council elections.
But the ward vote was postponed following the death on 15 April of Walter White, the Independent candidate who was also the sitting councillor.
Liberal Democrats in North Devon won 21 of the 41 seats up for grabs, gaining control of the council, that was previously run by the Conservatives, in coalition with the South Molton Independents.
New laws to improve conservation in the UK fishing industry are not working, according to evidence being heard at a parliamentary inquiry.

Walkers and cyclists are to get underpasses under the newly revamped North Devon Link Road.
The underpasses at Bishops Tawton and Landkey near Barnstaple follow the £93m widening of a 7.5km (5 mile) stretch of the A361.

The 23-year-old goalkeeper has been released by Exeter City after almost six years at St James Park.

Staff at a family attraction in Exmouth are receiving deaf awareness training ahead of the opening of a new £10m deaf academy in the town.
A group of students from the Exeter Deaf Academy have been visiting the Ocean Centre to help.
The academy wants to make Exmouth the UK's first deaf-friendly town.

The government says it is addressing the concerns of pupils at Ivybridge Community College, after a special Commons debate featured their experiences of exam pressures.
The debate was organised by South West Devon Conservative MP Sir Gary Streeter, who said he was sharing that students feared their mental health was being affected by exam-related stress.
Schools Minister Nick Gibb said the government was looking to reduce the pressures on students, in particular through better personal, social, health and economic education, external (PHSE).
He said students at Ivybridge "emphasise the importance of PSHE", and a new relationships and health education curriculum compulsory in all state-funded schools from September 2020 would be "probably the most significant preventative step of all".

The people of Helston have put on their finery and dancing shoes for one of the most important days in their calendar: Flora Day.
The dancing began at 07:00 and will go on into the evening.
The celebrations mark the end of winter and the coming of summer.

Firearms, cash and suspected drugs have been seized and four people arrested by police during raids in Devon and Dorset, police say.
Officers said, external a joint operation between the counties' forces involving more than 40 officers and separate firearms teams saw raids at six addresses in Charmouth and Axminster on Tuesday.
A 62-year-old man from Axminster was arrested on suspicion of conspiracy to supply Class A drugs and possession of a firearm before being released under police investigation.
A 49-year-old man from the town was arrested on suspicion of possession of a Class B drug with intent to supply and possession of a firearm. He was released on bail.
Two men from Charmouth, aged 19 and 34, were arrested on suspicion of conspiracy to supply Class A drugs. They were both later released under police investigation.

A former Royal Marine has been awarded more than £500,000 damages for hearing loss suffered during his service.
Alistair Inglis, 39, from Plymouth, said his hearing had been affected by weapon and vehicle noise.
He enlisted in 1997 for 22 years but left in 2012 "because of his hearing loss" a High Court judge ruled.
The Ministry of Defence (MoD) disputed the amount of damages he was entitled to, saying he left voluntarily while still "fit for all duties".

A 6ft (1.8m) high piece of Elvis artwork has gone missing from a fairground attraction in Devon.
The team at Dingles Fairground Heritage Centre said that, although it was not an old piece, it had "cultural and artistic significance" in the genre of fairground art.
Staff added that its return would be appreciated and there would be a reward for the person who handed it in.
About 250 passengers spent the night on board a Brittany Ferries ship in Plymouth's port after an engine problem prevented the sailing from going ahead.
Tuesday night's Amorique sailing from the city to Roscoff was cancelled and a further sailing was cancelled on Wednesday morning.
The ship Pont-Aven is also still out of action after a recent engine fire, which has led to cancellations, including Tuesday night's sailing between Portsmouth and Santander.
Brittany Ferries has apologised to customers for the inconvenience.
