A prison watchdog has looked into the self-inflicted deaths of 10 inmates at Glen Parva YOI and highlighted repeated failings at the jail.

They include lack of clear policy to protect and help vulnerable young men, failure to spot increased suicide risk factors and bullying not being investigated properly. 

The Prisons and Probation Ombudsman made his criticisms in his report into the death of remand prisoner Jake Foxall from Oxfordshire.
