Stafford hospital inquiry starts

A public inquiry begins on Monday into what happened at Stafford Hospital, where hundreds of patients are said to have died unnecessarily.

A 2009 report condemned conditions at the hospital, which it said were caused by senior managers putting targets ahead of patient care.
