A nursery temporarily shut after potties were not emptied and children were seen wandering aimlessly around has officially closed, Ofsted said.
The Cambridge Day Nursery in Milton Road was rated "inadequate" in all areas after an inspection in October.
The education watchdog said a later inspection in November found some improvements had been made, but issued a further list of requirements.
Ofsted said the owner had now resigned their registration and it had closed.
The day nursery in the city was temporarily shut on 11 October after inspectors from the Office for Standards in Education suspended the provider's registration.
Its report, external at that time cited a number of safeguarding concerns.
A nursery spokesperson said it was working hard to address concerns.
However, an inspection in November "found the provider had met some of the safeguarding and welfare actions but not others" and "another welfare requirements notice was served".
Ofsted has now said "the provider resigned their registration while suspended, external" on 6 December, and as such "we were not able to monitor the provider's response to meet the actions raised in the welfare requirements notice".
"They are no longer a registered childcare provider", the service said, and Ofsted's website, external lists the nursery as closed.
The BBC has asked the nursery to comment.
