A police investigation is under way after a woman died in hospital in Cumbria.
The 57-year-old, from Penrith. who has not been named, died at the Cumberland Infirmary in Carlisle on Sunday.
North Cumbria University Hospitals NHS Trust said a "patient safety incident" had taken place and an investigation was being carried out.
Cumbria Police said the coroner requested police investigate the circumstances surrounding the death.
A trust spokesman said: "We are saddened to confirm that a patient safety incident has taken place and we have declared this in line with our incident reporting process.
"The trust will conduct a full and thorough investigation.
"Our thoughts are with the family at this very difficult time."
