A prevention of deaths report has been issued to Aneurin Bevan Health Board after a patient died at the Grange Hospital

A student died after being neglected in hospital, a coroner said in a report.

Catriona Ellen Martin, 21, was admitted to the Grange Hospital, in Cwmbran, Torfaen, with autoimmune encephalitis on 5 December 2020.

It is a condition where patients suffer inflammation of the brain. It can be life threatening.

Coroner Caroline Saunders found Ms Martin did not get the one-on-one nursing needed and medicine was not administered correctly.

This resulted in Bristol-born Ms Martin, a student living in Cwmbran, developing dehydration, acute kidney injury and seizures. She died on 25 December, 2020.

Now Ms Saunders has issued a prevention of future deaths report to Aneurin Bevan health board.

She called the treatment given "unacceptable" and said Ms Martin died from autoimmune encephalitis "contributed to by neglect".

She said in the report: "The nursing staff relied on Catriona's mother to ensure that Catriona received the fluid and medication she required.

"Catriona's mother continuously advised the nursing staff that she was unable to ensure that Catriona was receiving fluid and medication, however no assistance was given, and Catriona went into a fatal decline.

"I found that Catriona's death would have been prevented if 1:1 (one-to-one) nursing care had been provided."

The health board must respond to the report within 56 days with details of a plan or explain why no action is needed.

A spokesman said it had received the report and would respond in due course.

"Our thoughts remain with the family of Catriona Martin," he added.
