Bournemouth forward Lee Tomlin is to have a medical at Bristol City on Monday, BBC Radio Bristol understands.
The 27-year-old looks set to return to the Championship club, for whom he scored six times in 18 games after arriving on loan in January.
Chief operating officer Mark Ashton said on 9 June that City were in "ongoing discussions" with the former Middlesbrough and Peterborough man.
Tomlin joined the Cherries from Boro for £3m in 2015 on a three-year deal.
He has made six Premier League appearances for Eddie Howe's side and scored in January's FA Cup third-round win at Birmingham City.
He became a Robins fans' favourite during his loan spell at Ashton Gate and head coach Lee Johnson praised the "maverick" as a "top-class" footballer.
Bristol City finished 18th in the Championship table last season, 12 points above the relegation zone.
