Wales could be without Jake Ball and Rhys Webb for their final autumn international against South Africa in Cardiff on Saturday, 2 December.
Lock Ball went off in the first half of Wales' 33-18 loss to New Zealand at the Principality Stadium with what appeared to be a serious shoulder injury.
Wales also lost scrum-half Webb before half-time after a blow to the head.
The Springboks have a worry over prop Tendai Mtawarira, who suffered a groin injury in their 35-6 win over Italy.
The tourists have called up Thomas du Toit, of Super Rugby side Sharks, as cover for his team-mate Mtawarira.
British and Irish Lion Webb must pass concussion protocols if he is to face South Africa.
"It [the Webb injury] wasn't an HIA [head injury assessment], it was just an immediate replacement from our medical staff," said Wales coach Warren Gatland.
"We knew he'd taken that knock. He wasn't assessed or anything like that, it's just they'd seen the incident and said that's a permanent replacement right from the start.
"He'll need to go through all the protocols for that."
During the autumn Wales have lost to both the All Blacks and Australia while narrowly beating Georgia, and will be looking to sign off on a high by claiming the scalp of South Africa.
