Hundreds of mental health deaths avoidable, inquiry finds

An inquiry from the Equality and Human Rights Commission has said the deaths of hundreds of people with mental health conditions, who were detained in police cells and psychiatric wards, could have been avoided.

Kathryn and Barry Wilson's daughter Rebecca committed suicide at the age of 18. The inquest into her death found that poor monitoring by her hospital gave her the opportunity to take her own life.

Poor planning, and transferring her from adolescent to adult psychiatric care, also worsened her problems.
