Government 'shocked' as NHS failure to probe deaths

The health secretary has said he is profoundly shocked by revelations that the NHS Southern Health Trust failed to investigate the unexpected deaths of more than 1,000 vulnerable patients.

Jeremy Hunt said the failures at the Trust, first revealed by the BBC, were "totally and utterly unacceptable".

The organisation is one of the largest mental health trusts covering five counties in southern England.
