'Missed chances' by NHS 111 to save baby who died from sepsis

A catalogue of failures led to the death of a baby boy from sepsis, according to a damning official report, after William Mead died in 2014.

An NHS England investigation found that he might have been saved if call handlers on the emergency 111 helpline had recognised the seriousness of his condition.
