Keanu Williams: 'Radical change needed' - Safeguarding Children Board

There were "a number of significant missed opportunities" to save a two-year-old boy from being beaten to death by his mother, a report has found.

Keanu Williams died after suffering 37 injuries, including a fractured skull and torn abdomen in January 2011.

His mother, Rebecca Shuttleworth, 25, was convicted of Keanu's murder and is serving a life sentence.

A serious case review found police, social services and health workers "failed" to prevent his death.

Jane Held, from the Birmingham Safeguarding Children Board, said "radical changes" were needed as "professionals feel unable to challenge other professionals from a different agency."
