Red Arrows pilot Sean Cunningham's father says his death 'was preventable'

The family of a Red Arrows pilot who died after being accidentally ejected from his grounded aircraft said his "tragic death was preventable".

Flt Lt Sean Cunningham was killed at RAF Scampton in Lincolnshire in 2011.

Coroner Sean Fisher concluded a number of safety issues with the ejection seat had gone unnoticed and a tightly fastened bolt had caused parachute failure.

Speaking after the inquest Flt Lt Cunningham's father Jim Cunningham said: "We still find it difficult to accept that so many people missed... what should have been obvious to those having a duty to ensure his safety."
