Grandfather death was avoidable says Anna Soubry MP

The MP for a pensioner who was stabbed to death by his schizophrenic grandson has said an independent report into the case showed a catalogue of failures by the trust responsible.

John McGrath, 81, was killed in July 2009 at his home in Stapleford by 32-year-old William Barnard who had stopped taking his medication.

The report said Nottinghamshire NHS Healthcare Trust missed many opportunities to section Barnard under the Mental Health Act.

MP Anna Soubry said: "John McGrath's death was completely avoidable."

The trust has apologised to the family and said it has acted on all five recommendations in the report.
