Slow Welsh ambulance times failed Pontypridd men
The ambulance service has been accused of systemic failure after two cases of not reaching emergencies on time despite available vehicles.
Public Service Ombudsman for Wales Peter Tyndall upheld two complaints and has made nine recommendations.
Mr Tyndall spoke to Oliver Hides on BBC Radio Wales' Good Morning Wales programme.
