Fighting intensifies near MH17 crash site

Fresh fighting, close to where the Malaysia Airlines jet went down in Ukraine, has prevented international monitors from accessing the crash site.

It comes as security officials in Ukraine say the plane suffered an explosive loss of pressure after it was punctured by shrapnel from a missile.
