The murder of a mother-of-three by her estranged husband could have been prevented, a multi-agency review says.
Sana Shah suffered years of abuse from her husband, Anwar, who later admitted killing her at the home they had shared in Wolverhampton in March 2014.
Risk assessments about her welfare were poor, or not picked up resulting in interventions not being made, the report said.
"The report is clear that professionals worked hard to support the victim but, with the benefit of hindsight, some opportunities for agencies to put in place interventions to protect her were not taken," the Safer Wolverhampton Partnership said.
The Domestic Homicide Review changed the names of the couple to protect the identity of their children.

Officers say they're not looking for anyone else in connection with two bodies discovered in a house in Birmingham.
Police were alerted after paramedics went to an address in Raglan Way, Chelmsley Wood, at about 09:30 and found the bodies of two women.
Officers said they were treating their deaths as unexplained.

Fourteen offenders linked to the Burger Bar Boys gang in Birmingham may have their prison sentences increased after a referral today which saw their cases sent to the Court of Appeal.
Earlier this year they received prison terms after being convicted of supplying guns to criminal gangs around the country.
Nosakhere Stephenson was the head of the group - he and the rest of them were sentenced in November.
Today the Solicitor General referred the sentences of all offenders to the Court of Appeal under the Unduly Lenient Sentence scheme.
The announcement of Walsall, external's new manager is imminent.

The Midlands Metro tram line is to be extended from Birmingham city centre to the airport and from Wednesbury to Brierley Hill, under a 20-year vision for transport in the West Midlands agreed today.
West Midlands Integrated Transport Authority have also agreed to introduce a rail service between Walsall and Wolverhampton and build new stations at Willenhall and James Bridge in Darlaston.
In Birmingham the plan is to have new suburban rail lines and stations for northern suburbs like Castle Vale and Castle Bromwich and Moseley and Kings Heath in the south.
Up to 100,000 Walsall householders are set to lose their weekly bin service next year, external in a bid to save the council more money.

Two men who worked at a company in the Black Country have died after a lorry and a van crashed on the M6 this morning.
The collision happened at about 05:30 on the southbound carriageway between junction 1 for Coventry and 2 for Rugby.
Insp Julian Smith, from Warwickshire Police, external, said: "We know that they worked for a local company, we believe in the Walsall area.
"They left in the early hours and they were off to make some deliveries prior to being involved in this collision."
Burglars raided a Lichfield business, external in the early hours of Thursday morning.

The bodies of two women have been found at a home in Chelmsley Wood.
Police were alerted by ambulance colleagues at about 09:30 to concerns for two women found inside a home in Raglan Way.
Their deaths were being treated as unexplained, West Midlands Police, external said.
Post mortem tests will take place in due course to establish a cause of death. The force said it was not seeking anyone else at this stage.
