  7. Shropshire hospitals report: 'Awful read for those who lost a child'published at 07:56 Greenwich Mean Time 29 November 2018

    Michael Buchanan
    Social Affairs Correspondent, BBC News

The damning report into services at Shrewsbury and Telford Hospital Trust will be an awful read for the families who have lost babies as a result of errors.

Speak to any of them, and their fervent wish is that nobody has to go through the same experience as they did.

The continuing inability of the trust, therefore, to ensure heart rates are properly monitored and the failure to learn lessons after incidents, smacks of a callous arrogance towards those families.

Fundamentally this is a trust that has been badly led for years.

Today's report is stark in its denunciation of the current management - not all of them have the "right skills and abilities" to provide "high-quality sustainable care."

Chief executive Simon Wright insists he is not out of his depth, but under his watch the trust has demonstrably deteriorated.

  8. Ten-goal thriller at Villapublished at 07:39 Greenwich Mean Time 29 November 2018

Forest squandered leads on three occasions, while Villa only led once in the entire match - yet still the teams could not be separated after 90 minutes.

At the end of a breathless encounter, the two sides remained three points apart in the Championship table, with eighth-placed Villa still unbeaten at home since Dean Smith's arrival on 10 October.

Villa have taken 13 points from a possible 21 since he took charge.

Jaydon James, 16, known as JJ, was wounded in Coventry on Saturday night, and died later in hospital.

He was attacked while with friends in Deedmore Road, Wood End, a suburb four miles north of the city centre.

Two of his friends were also seriously injured in the attack.

A book of remembrance has been opened at St Chad's Church in Hillmorton Road which is hosting the service in memory of Jaydon from 19:00.

  10. Shropshire baby deaths: Hospital trust rated 'inadequate'published at 07:18 Greenwich Mean Time 29 November 2018

A hospital trust which is being investigated over a cluster of deaths and injuries to babies has been rated inadequate.

    Princess Royal Hospital

A 112-page report, published by the Care Quality Commission, lists a catalogue of issues, including a culture of bullying and harassment, at the Royal Shrewsbury Hospital and Princess Royal Hospital in Telford, Shropshire.

Shrewsbury and Telford Hospital Trust, which runs the two sites, is already in special measures and emergency and maternity services are deemed unsafe.

Chief executive Simon Wright apologised and said the trust was "disappointed that we have not made as much progress to tackle the issues and challenges that the trust faces as we all want".

The trust was previously rated as "requires improvement" overall.
