A police force has been criticised for a string of failures in its investigation into the harassment of a woman who went on to kill herself as a "direct result" of her ex-boyfriend's controlling behaviour.
Nicholas Allen was jailed for 10 years in July 2017 after what then was thought to be the first manslaughter case brought in such circumstances.
The trial heard Allen, 47 at the time, sent Justene Reece, from Stafford, abusive voicemails, texts and Facebook messages and stalked her.
The police watchdog, the Independent Office for Police Conduct (IPOC), said it started probing Staffordshire Police's response to 34 incidents either directly involving or connected to Ms Reece. , external
It said it found the force consistently failed to connect cases together with previous ones, broke its own policy in some incidents and didn't put together the full scale of Mr Allen's offending.
The IPOC said Staffordshire Police is going to give officers better training in future and several staff are being talked to about poor performance in their jobs.

Staff said Juliet Scroggie, five, needed a doctor's certificate because of 'increased risk of injury'.

Stoke City striker Sam Vokes says the side have to turn more draws into wins if they're to challenge for promotion next year.
This season, the Potter have lost one game more than third-placed side Leeds United but have drawn 21, the most of any side in the Championship.
Their latest stalemate, 0-0 away at Millwall, left Stoke 16th and with no chance of climbing a further place in the division with just one more game to go.
Quote MessageIt is about turning some of those draws into wins and I think our loss percentage is up there with the top three or something so it's those draws probably where we've dropped the points this year. We're not far away, turn a few of them around and we could be pushing next season, definitely."

A wildlife centre says it was a "hard decision" to put a bald hedgehog to sleep despite an initial promising recovery.
The hedgehog, who staff named Bear, was found in a Shropshire garden and taken to a wildlife centre in January after being found with hardly any spines.
Stress-busting massages led to its prickles growing back and plans were made to release it later in the year.
However, a spokesperson for Cuan Wildlife Rescue said Bear's health deteriorated and had to be put to sleep on Friday.
Quote MessageWe had given him so much, and so many of you out there had been wishing him well, it was a hard decision to take, but it was very evident that to have kept him alive would have caused him suffering and distress."

Worcester Warriors' director of rugby says it was "absolutely vital" that the team secured their Premiership survival at the weekend and kept their fate in their own hands.
They were trailing at home at half-time against Gloucester, but tries from Niall Annett and Darren Barry gave them a 27-20 victory in the end.
The victory leaves Worcester 11 points clear of bottom side Newcastle with 10 left to play for.
Quote MessageWhen we perform well, we give ourselves every prospect of the result. It was absolutely vital we kept things in our own hands. The players would have known exactly the situation, and I just felt if we played really well and were emotionally there, the rest would hang together."
Alan Solomons, Worcester director of rugby
