    A senior nurse at a psychiatric hospital where a woman killed herself says he repeatedly told management he felt staffing levels were not safe.

    An inquest found failings in Claire Greaves's care at Cygnet Hospital, Coventry, including insufficient staff, contributed to her death.

    Ms Greaves, 25, from Pontypool, had anorexia and a personality disorder.

    Cygnet said staffing levels were always run at nationally recognised safe levels.

    Four arrests have been made after a man was shot in the foot in Wolverhampton.

    The victim, in his 30s, suffered the wounds on Joan Street yesterday afternoon, police have said.

    Four men were arrested on suspicion of firearms offences and are still in custody.

    Telford and Wrekin Council has made five promises to residents following the local elections earlier this month.

    The Labour group strengthened its grip on power with an increased majority and said it would stick to its pledges between now and the next set of elections.

    They council says it will:

    • Fight for services at the Princess Royal Hospital
    • Keep council tax low
    • Spend £80m a year on vulnerable adults and children
    • Spend £50m a year on roads and footpaths
    • Fight plans to merge the authority with Shropshire Council

    The Royal Horticultural Society (RHS) event is seen as the pinnacle for many UK gardeners.

    Among those to get the highest honour is Birmingham City Council, winning their eighth gold with a climate change-inspired garden. , external

    Other gold winners include Worcestershire-based Hardy Eucalyptus, Shropshire's David Austin Roses and the Family Monsters Garden, designed in Coventry.

    West Midlands Ambulance Service says it's going to be relying less on motorbikes and response cars and more on traditional ambulances in the future.

    Mark Docherty, its executive director of clinical commissioning, said: “We invested a lot in response cars, a lot of motorcycles and even bicycles. But what we found was that it was all well and good getting a paramedic there, but what do they then do with the patient?"

    Mr Docherty said he also wanted more defibrillators "on every street corner", so that if someone suffered a cardiac arrest, they could get help quickly.

    An £11m scheme to widen a bridge won't finish for at least another month, with the delay being blamed by a council on poor weather and utility firms.

    Sydney Road bridge in Crewe, Cheshire, was shut at the end of October, external so the crossing could be replaced with a wider structure to carry traffic in both directions.

    But Cheshire East Council says bad weather earlier this year and delays with work carried out by utility firms has seen the re-opening date slip from the end of this month to the end of June., external

    Shrewsbury Town's Sam Ricketts says he wants his players to be fit when they report back for pre-season training.

    He felt their standards had slipped when he took over in November and that was having an effect on performances.

    He said: "When I first came in we were the team conceding late goals, the longer the season went on, it was us scoring late."
