The departure of the chief executive of Shrewsbury and Telford Hospital Trust has been raised in Parliament today with Telford MP Lucy Allan saying he had presided over a "catalogue of failings".
The hospital trust is currently in special measures over failings in the maternity and emergency departments, but the most vocal criticism has perhaps been regarding the planned reorganisation of hospital services.
On Monday, Simon Wright said leaving had been a difficult decision, and, despite recent challenges "the foundations have been laid to build a better future".
He is leaving the hospital trust to take up another post at Nottingham and Nottinghamshire Integrated Care System.
Speaking in Parliament, Telford MP Lucy Allan called for senior NHS management to be better held to account.

Shrewsbury Town have sold goalkeeper Steve Arnold to Northampton Town for an undisclosed fee.
He had established himself as the club's first-choice goalie last season, but had asked to move closer to home for family reasons, saying, "when you are at a club a long way from your family you sometimes have to make tough decisions."
Manager Sam Ricketts said the player came to see him at the end of the season, requesting a move and he said: "As a club what we have done is try and help him, we all want to play football and enjoy that, but you also need to be happy family-wise away from the field."

There are a number of candidates to be Crewe Alexandra captain next season, assistant manager Kenny Lunt believes.
He said: "We've got a few and that's what we wanted, especially through the spine of the team. We're not worried by how many leaders we've got now."
New signing Ollie Lancashire is one of those in the frame to become skipper after George Ray left the club and Lunt said "we've got a good changing room which showed last year".
He said that showed in their ability to fight back and score late goals last season.

A senior hospital consultant working in Telford has welcomed the resignation of the Shrewsbury and Telford Hospital Trust chief executive, Simon Wright.
Staff members said an extraordinary meeting was due to take place on Monday, with plans to discuss a vote of no confidence in him.
One member of staff said: "There’s an awful lot of staff who are hugely relieved that he’s going and they hope that someone new will look at the grassroots problems with the trust so we can care properly for patients.”
Plans for a £2.5m hockey centre in Worcester have been approved by the city council.
Councillors agreed to lease the land on the Old Porcelain Ground off Droitwich Road to the Royal Grammar School and Worcester Hockey Club for a nominal fee.
The facility, which would also be used by schools and the wider community, will still need to secure planning permission.

Plans for homes, businesses and a school on the site of a former power station could take decades before they're built.
That's assuming proposals, just submitted by the owners of Rugeley Power Station, Engie, get permission from Cannock Chase Council and Lichfield District Council.
The planning application's now been submitted to both authorities as, given the size of the land, it straddles both districts.
If the outline plans get approved, work could begin in 2020 but take until 2040 to finish as it would be carried out in stages.
A man has died in hospital after being knocked off his scooter in Sutton Coldfield earlier this week.
The collision, which also involved a Ford Ranger happened on the A4091 at the junction with Bodymoor Heath Lane, late on Monday evening.
Warwickshire Police said the 21-year-old man from Tamworth died in hospital this morning.

Herefordshire Council says it will consider the proposals and Michael Leighton wrote: "Brilliant idea. Replace cars with trams all over the city imagine how cleaner the air would be."
But others were critical. Christopher Craven commented: "What a ludicrous idea. Have we lost the ability to put one foot in front of the other? These aren't vast distances. Having to sit and wait for the tram to pass will only make traffic issues worse."
Louise Ingram wrote: "Hereford needs a bypass. A tram would not run late enough and would never be on time."
And Sylvie Manns added: "We have Hopper buses that run into the city centre from all areas of Hereford so that people do not have to use cars, so not sure how this would relieve congestion. People who prefer to drive will still drive."
Firefighters have hitched a lift on a steam train to get to a fire near a railway station in Staffordshire.
Staffordshire Fire and Rescue Service tweeted to say they were dealing with an incident involving a burning gas pipe close to Froghall Railway Station.
The firefighters are working with gas engineers to deal with the disused pipe.

A council's admitted it could take years to set up an official transit site for travellers.
It comes as one group of travellers who were evicted from Wolstanton Marsh, Newcastle-under-Lyme, yesterday have moved to a nearby patch of open land in Clayton.
The council and Staffordshire Police said they were working on getting another court order to move them on. , external
But, overall, the borough council has said it's faced local opposition over any plan to find an official site for them.

BMW and Jaguar Land Rover (JLR) are to join forces on developing electric car technologies.
The car giants said they would work together to develop electric motors, transmissions and power electronics.
As part of the partnership, Jaguar Land Rover, which has headquarters in Coventry, said it would produce electric drivetrains at its Wolverhampton plant, which employs 1,600 people.
Both firms have struggled to maintain profit margins amid falling car sales and higher costs as well as the need to invest in future technologies.
Car firms are being forced to make low-emission vehicles to meet more stringent anti-pollution rules.
