    The health care expert leading the inquiry into maternity failings at two Shropshire hospitals has said no apology will be adequate for the families affected.

    "Many families have described to me how they live on a daily basis with the results of that poor care," Donna Ockenden said.

    Ms Ockenden said the leaked interim report, seen by The Independent, was not meant for publication.

    She said the families affected want "one, single, comprehensive independent report covering all known cases of potentially serious concern within maternity services at the trust. My independent review team and I are working hard to achieve this."

  2. College closed to visitors after 'suspicious incident'published at 14:47 Greenwich Mean Time 19 November 2019

    The Buslem campus of Stoke-on-Trent College will remain closed to visitors for the rest of the day after what the police called a "suspicious incident".

    Officers were called to the site at 11:30 to help staff restrict entry to the site "as a precaution", but neither the police nor the college will say what the concern was about.

    Police have now left the site and the college said lessons and exams continue as normal today.

  3. Babies left brain-damaged at hospital trust - reportpublished at 14:30 Greenwich Mean Time 19 November 2019

    Cases reviewed at the Shrewsbury and Telford Hospital NS Trust included 22 stillbirths, three deaths during pregnancy, 17 deaths of babies after births and three deaths of mothers.

    An interim report, leaked to The Independent newspaper, pointed to:

    • Babies left brain-damaged because staff failed to realise labour was going wrong
    • Failure to adequately monitor heartbeats
    • Babies left brain-damaged from infections that could be treated by antibiotics

    The report was ordered by the government in 2017, following the efforts of two couples who had lost their babies shortly after birth.

    Rhiannon and Richard Stanton Davies lost their daughter Kate in 2009, and Kayleigh and Colin Griffiths lost their daughter Pippa in in 2016.

    There have been major failings at a Shropshire health trust for 40 years and lessons are not being learned, a leaked report shows.

    Children were left with permanent disability because of care at the Shrewsbury and Telford Hospital Trust, an inquiry, led by maternity expert Donna Ockenden, shows.

    Staff at the trust routinely dismissed parents' concerns, were unkind, got dead babies' names wrong and, in one instance, referred to a baby who died as "it".

    The report, which was set up to examine 23 cases is now looking at more than 270 since 1979.

    The Duke of Cambridge will visit West Bromwich Albion later this month, Kensington Palace has announced.

    He will meet members of the first team to talk about some of the pressures they face as players, as part of the Heads Up campaign, it said.

    The mental health initiative, which is particularly aimed at men, was launched at the Community Shield in August 2019 and will culminate at the 2020 FA Cup final next May.

    It's being run by the Football Association in partnership with mental health charity Heads Together.

    Stoke City manager Michael O'Neill will take charge of Northern Ireland again tonight for their final Euro 2020 qualifying game - against Germany.

    His work with Northern Ireland won't affect his preparation for Stoke's game against Wigan on Saturday,he said, and explained all the preparation and scouting for games was done well in advance.

    O'Neill also pointed out their opponents won't be playing during the international break, so no scouting can be done and added: "I could watch 40 games, or watch two games and pretty much my squad will be 99% the same."

    Northern Ireland can't qualify automatically for the European Championships, but O'Neill will take charge of the team for their playoffs in March.

    A 44-year-old man who was arrested on suspicion of murder has been released on bail.

    He was arrested a woman died in Upton-upon-Severn on Saturday.

    A man who was found dead after being missing for several days has been described as having a smile that "lit up the darkest of days".

    Ronald Lovell , 91, was reported missing from his home in Old Milverton, Warwickshire, on 13 October.

    His body was recovered from the River Avon on 26 October.

    His great niece Rachael said: “Uncle Ron was the kindest soul you could ever meet, a true gentleman and an inspiration to us all."

    All rail lines have reopened following damage to the overhead electric wires between Stafford and Wolverhampton, the National Rail Enquiries website reports, external.

    However, services running between these stations may still be cancelled or delayed and disruption is now expected to last until 12:30.

    A Worcestershire pub which is still surrounded by floodwater is offering to ferry its customers in and out.

    The Camp House in Grimley has been cut off since last week's heavy rain, with the beer garden and car park completely underwater.

    The ferry service runs between 12:00 and 16:00 but you'll have to bring your own wellies to get on and off the boat.

    Thieves used a 4x4 to steal a cash machine from a petrol station outside Nantwich.

    Two men put a rope round the machine at Wardle service station during the early hours of Monday and dragged it free before driving off towards the town.

    Cheshire Police said the dark coloured 4x4 had a smashed rear window and broken tail light and are asking people along the road for CCTV footage.

    Shrewsbury Town face an FA Cup replay against Bradford City tonight and midfielder Dave Edwards said they would be focused on winning this game, instead of dreaming of ties against big clubs in later rounds.

    He also insisted their League Two opponents shouldn't be underestimated and said: "It could be a lot worse, a replay away at Bradford. It's a nice stadium and they're a good, good team, so we're going to have to be up for it."

    A trainee police dog has been funded directly by the proceeds of crime.

    Poca, a ten-week-old cocker spaniel has been named after the Proceeds of Crime Act, which seeks to claw back money made from wrong doing.

    The puppy will live at home with her handler, PC Kaz Richards, and is being trained to become a specialist detection dog, the force added.

    Poca is the first Warwickshire Police dog to be funded in this way. A spokesman said the money used to buy her from a private breeder was forfeited by the courts under the Misuse of Drugs Act and amounts to money seized from convicted drug suppliers.

    The first phase of a major 800-home plan for former green belt land in Coventry has been recommended for approval.

    The land in Keresley was taken out of the green belt and allocated for 3,100 homes under the city's local plan two years ago.

    Outline planning permission for up to 800 homes, a primary school, a local centre, and public open space was granted last year.

    Developer Taylor Wimpey has now submitted details for 322 homes on land off Tamworth Road. The plan has been recommended for approval ahead of a meeting on Wednesday, 20 November, external.

    A man and a woman have been charged with murder after a man was fatally stabbed.

    Mazammal Mahmood Butt, 27, was found seriously injured in Lozells Street, Birmingham, on Sunday afternoon, and died before arriving at hospital.

    He died from a single stab wound, police said.

    Mobeen Shahzad, 27 of Osborne Grove, Lozells and 28-year-old Sheridan Fitzsimmons, of Conybere Street Highgate are due to appear at Birmingham Magistrates' Court later.
