  5. Driver died 'while trains were being moved'published at 18:52 Greenwich Mean Time 16 December 2019

    A train driver who died in a "serious incident" at a Birmingham depot on Saturday was injured "while trains were being moved," West Midlands Ambulance Service said.

    Paramedics were called to the maintenance depot in Tyseley just after 20:00, "but sadly it became apparent nothing could be done to save the man and he was confirmed dead at the scene," a spokesperson said.

    The death will be investigated by the Rail Accident Investigation Branch, a West Midlands Trains spokesman said.

    He added that no members of the public were involved and there was no follow-on disruption to passengers.

    Work is yet to begin on signing off Worcester's already delayed new railway station, the rail regulator says.

    Despite suggestions it was already carrying out safety checks at the £22m station, the Office for Road and Rail (ORR) said it was still waiting for a report from an independent assessor employed by the station’s builders.

    Trains were due to start calling at Worcestershire Parkway, near junction seven of the M5, this month, but passengers have already been told they will not be able to travel to or from there until "early next year".

    However, on Friday the county council admitted it couldn’t give a precise date for its opening, adding delays were due to a weak embankment and queries from Network Rail.

    When the regulator finally gets the report, it said officials should take another month to authorise the station.

    While most eyes were focussed on the election results on Friday, the latest A&E performance figures were released, external, and paint a bleak picture.

    The BBC's Hugh Pym said, external A&E departments in England reported their "worst ever levels" in November, with just 81.4% of patients seen within the four-hour target.

    Some West Midlands hospitals' A&E units continue to be among the worst in England.

    Shrewsbury and Telford Hospital Trust - currently in special measures in part due to its emergency care - treated just 65.6% of patients inside four hours.

    University Hospitals Birmingham NHS Foundation Trust's figures were even worse at 64.7%

    Managers at the under-fire Shrewsbury and Telford Hospital NHS Trust (SaTH) say they are "working very hard to change the culture of the organisation”, and hope a national standards body will reflect improvements next year.

    The Care Quality Commission (CQC) has been inspecting services at the Princess Royal Hospital, in Telford, and the Royal Shrewsbury Hospital since 12 November, and is expected to publish its findings in spring 2020.

    The trust is at the centre of an independent inquiry into concerns over maternity care and has been in special measures since November 2018. Emergency services also require urgent action.

    But Beverley Tabernacle, the trust's transformation and strategy director told a Shropshire-wide health committee the initial post-visit feedback, though “mixed”, acknowledged improvement in some areas, including maternity.

    But she added that she couldn’t say whether the coming report would show an improvement on the CQC’s last trust-wide report, which rated SaTH as “inadequate” overall.

    Earlier we heard of signalling problems around Birmingham's Moor Street Station.

    According to National Rail Enquiries, it seems the problems may be more widespread, with commuters warned of lengthy delays on services run by several operators through central Birmingham stations.

    "Multiple signalling issues" in the city are causing disruption with trains running to and from Birmingham stations, with services facing delays and cancellations until at least 21:00, National Rail Enquiries said.

    There is also a shortage of West Midlands Railway train staff which is causing additional disruption to its services through Birmingham.

    Services run by Avanti West Coast. Chiltern Railway, Cross Country, London Railway, Transport for Wales and West Midlands Railway are affected.

    A man has been arrested after armed police were called to a property in Shelton in Stoke-on-Trent following reports a man had a firearm.

    The incident on Harcourt Street began at about 15:00 on Sunday lasted for more than an hour, Staffordshire Police said.

    Trained negotiators were called to the scene and a man was subsequently arrested on suspicion of affray.

    He's been released under investigation pending further enquiries, the spokesman added.

    A man has been jailed for 25 years after he stabbed stabbed a sex worker in the neck and chest in a "random" and "unprovoked" attack.

    Bartosz Konopacki, 28, of Hilmorton Road in Coventry, was found guilty of attempted murder at Warwick Crown Court after the attack in the Hillfields area of Coventry in the early hours of 7 May.

    West Midlands Police said it is understood Konopacki split with his partner shortly before the attack and "was intent on taking out his aggression on any woman he could find".

    His victim eventually made a recovery, but has been left scarred for life, police said.

    A Bridgnorth care company has been put into special measures after being rated as inadequate by inspectors.

    A catalogue of failures were listed by the Care Quality Commission against Shropshire Crewe, run by Alpha Support Limited, which looks after people in their own homes.

    Complaints included allegations of "rough handling", a staff member sleeping on duty and late and missed calls. Record keeping and evidence of staff training were also criticised., external

    "People who used Shropshire, Crewe did not receive a safe, effective, caring, responsive or well led service," inspectors said.

    The visit was prompted by a complaint by a local authority quality assurance team.

    "Services in special measures will be kept under review and, if we have not taken immediate action to propose to cancel the provider's registration of the service, will be inspected again within six months," inspectors concluded.
