Part of the A441 Pershore Road remains closed in both directions after the crash at the junction of St Stevens Road shortly before 05:30, said the ambulance service.
The car ended up against a building with both vehicles suffering significant damage.
Two teenagers in the car were taken to hospital, along with the driver of the van - a middle-aged man.

More than 1,500 people have signed a petition to save an old Birmingham library building.
Selly Oak Library was given to the city in 1905, but it closed in 2017.
Members of the Selly Oak Community Development Trust want the council to hand them control of the building, so they can turn it into a new arts centre.
Two people have died after a car hit a tree near Congleton in Cheshire East.
Police discovered the vehicle on Congleton Road, Marton, at about 03:00 on Sunday.
The two occupants of the Mini Cooper died at the scene, said the Cheshire force.
Formal identification has not yet taken place but they are believed to be a 22-year-old man and a 32-year-old woman from the area, the force adds.
