The NHS "failed" to properly investigate the health service blunders that led to the death of a boy from sepsis, a report has found.
Three-year-old Sam Morrish, from Devon, died from the treatable blood poisoning condition, external in 2010 because four health service organisations made mistakes, the Parliamentary and Health Service Ombudsman (PHSO) previously found.
The PHSO found the local NHS processes "were not fit for purpose" and excluded the family and junior staff. The health watchdog has called for a national accredited training programme for people carrying out NHS investigations.
