    An independent investigation has been ordered by health commissioning group NHS Kernow of its own procedures after failures in the care system were exposed by a BBC Panorama programme

    The home at the centre of the scandal - Clinton House - has now been closed but Cornwall Council and the NHS continue to fund some of the 200 or so residents still housed in five other homes also run by the Morleigh Group. The has group apologised.

    Dr Iain Chorlton chairs the board of the clinical commissioning group. He said capacity issues in the NHS were at the root of the problem, and it could only be tackled with a change of attitude from both doctors and patients.

    • In Cornwall, there are reports in Helston of a stuck coach at the bottom of The Furry. Approach with care 
    • There's slow traffic on the A38 and A388 near Saltash at the Carkeel roundabout 
    • In Devon, one lane is blocked on Tavistock Road in Plymouth, inbound, at The George junction due to a broken-down vehicle. Queues are back to Roborough Down 
    • In Exeter, there are reports the traffic lights have failed on Exwick Road at Buddle Lane
    • From Kennford, heading towards Exeter, there's slow moving and queueing traffic on the A38 and M5 
    • In Hatherleigh, the A3072 is partially blocked near Moor View due to an incident

    The father of a young Devon woman who died playing rugby is using the anniversary of her death to urge more people to sign up as organ donors.

    Twenty-two-year-old zookeeper Lily Partridge suffered a fatal head injury in a tackle during a training session at North Tawton, Devon, a year ago today. She carried two donor cards, which have meant four people have been saved directly and dozens of others have been helped. 

    The South West Organ Donation and Transplant Service said it saw a "significant increase" in people registering to become donors after her death, dubbing the rise of "the Lily effect".

    Her dad, Jeff Partridge, said he hoped a rise in donors meant some good had come from her death.

    Developers have revealed plans for a new five-star hotel and apartment block on a prime site on Plymouth Hoe (below).

    The city council said it had chosen Henley Real Estate as the preferred bidder to develop the land. Its £50m plans include a boutique hotel and 80 flats, and work could start at the end of next year.

    The authority bought and demolished the former Quality Hotel (above) which had been derelict for years and suffered a number of arson attacks.

    A Devon council has been accused of putting a dampener on the Christmas spirit by banning wreaths and other Christmas decorations from the communal areas of its properties. 

    Residents in East Devon have been told the decorations are a fire hazard.

    East Devon District Council said communal areas had to be kept clear of hazards and it was trying to keep residents safe.

    The 12 former and current students were crowned winners of The Choir: Gareth's Best in Britain on BBC2 last night.

    The all-male group was recognised as the nation's most talented and entertaining amateur choir.

    Police have concluded no criminal offences were committed in the fire service's delayed response to a fatal blaze in Plymouth.

    It took firefighters more than an hour and a half to reach the home of 76-year-old Molly Wigmore in October.

    Devon and Cornwall Police investigated after a complaint about the long delay. Devon and Somerset Fire and Rescue Service said it was carrying out its own internal inquiry.
