A serious case review is under way following the death of a four-week-old baby in St Blazey, Cornwall, last year.
Cooper Johnson Hicks died of head injuries and a coroner ruled yesterday that he was unlawfully killed by his father, Tristan Hicks - who took his own life after he was questioned on suspicion of murder.
Sarah Allum from the NSPCC says the police and social workers who were involved in the case before Cooper's death need to learn lessons.
"All agencies will have to review all of their involvement with this family with this young baby and to look at all actions that took place and of course any omissions, and consider what might have been done to protect this baby," she said.

Torquay United loanee Jordan Lee is undergoing tests after suffering chest pains.
The 20-year-old right back, on loan from Bournemouth, went to hospital with club physiotherapist Gareth Law on Wednesday and now faces some time out.

A yacht with 10 people aboard taking part in the Rolex Fastnet offshore race had to be rescued yesterday afternoon after getting into trouble.
The RNLI said Vencom lost its rudder and was unable to steer so had to be towed back to shore with help from both St Mary's lifeboat from the Isles of Scilly and Ballycotton all-weather lifeboat based in Ireland.
Image source, RNLIThe notoriously tough race on Sunday saw 380 boats sail from the Solent round the Fastnet lighthouse off Ireland and back to the finish line in Plymouth.
Ballycotton RNLI Coxswain Eolan Walsh said: "This was the furthest offshore rescue that I personally have ever been requested to.
"We would like to commend both the crew members on board the yacht who were all wearing suitable lifejackets and also thank the crew on board the St Mary's lifeboat for their efforts in ensuring a positive outcome."

The Football Association has said Plymouth Argyle defender Sonny Bradley has "no case to answer" after reviewing video evidence of an alleged spitting incident against Peterborough on Saturday.
After the match, Peterborough player Marcus Maddison claimed he had been spat at while on the floor injured.
It means Bradley will serve no suspension and will be available as the Pilgrims host Charlton in League One this weekend.

The Labour leader Jeremy Corbyn has said Cornwall is "absolutely in the party's sights" for the next general election, on a visit to the county.
Mr Corbyn is holding a rally on the NHS at Heartlands in Pool later, as part of efforts to raise Labour's profile in target seats.
The party made significant gains in the 2017 general election but the Conservatives held onto all six parliamentary seats.
After a tour around the Royal Cornwall Hospital in Truro he said a Labour government would "particularly look at the source of problems" for pressure on the NHS.
"The social care is not available so they [patients] are stuck in hospital, as a result somebody else cannot go in hospital, as a result somebody else can't get elective social care," he told us.
The Conservatives have pledged £2bn for adult social care in England over the next three years.

Canada international Matt Evans says he hopes to return to the Cornish Pirates side in November after a knee problem.
The 29-year-old wing has been out since December, and has had two operations on the injury.

There's been a dramatic turn around at a Devon hospital according to the latest Care Quality Commission report.
Last year inspectors said the Emergency Department at Torbay Hospital was overwhelmed and rated "inadequate".
The hospital's made a number of changes since then including improving the rota cover of senior doctors and nurses seven days a week. It's now rated "good".

Two men have been jailed for life for murdering a drug user whose body was found dumped in a wheelie bin.
Drug dealer Thomas Killen, 33, and addict Brett Edwards, 37, attacked James Woodhouse, 30, over a £700 cocaine debt and left him dying in a squalid flat in Exeter, the city's crown court heard.
He was knocked unconscious and died from an injury to his neck which he suffered as he fell over, the court was told.

Killen, of North Lawn Court, and Edwards, of Okehampton Road, both denied murder but were found guilty.

Football is remembering former Exeter City striker Adam Stansfield, who died of cancer at the age of 31.
The English Football League tweeted, external: "Seven years on. Adam Stansfield; gone but never forgotten."
He was diagnosed with bowel cancer following a series of tests for an abdominal complaint in April 2010, after being admitted to hospital in March.
More than 1,000 mourners attended his funeral.

Paul Taylor, from Torquay, has been missing since 25 July.
Paul’s family are getting "increasingly concerned" and want to know if he is safe and well, police said.

The boss of the new South Western rail franchise has refused to commit to protecting the role of train guards, despite calls from the RMT trade union.
FirstGroup and Hong Kong's MTR take over on 20 August and will provide 90 new trains with doors that can be operated by the driver.
Driver-only operated trains on Southern Rail services has led to 16 months of industrial action by the RMT. The union has called a meeting with FirstGroup on 17 August.
The South Western rail service currently operates routes between London Waterloo, Reading, Bristol, Exeter, Weymouth, and Portsmouth, as well as Island Line on the Isle of Wight.

The NSPCC says lessons must be learned after a four-week-old baby was "unlawfully killed" by his father.
A coroner in Truro ruled Cooper Johnson-Hicks was killed by his father, Tristan Hicks, who had a history of mental health problems and domestic violence.
Mr Hicks took his own life a month after Cooper died in June 2016.

Two men have been found guilty of killing a drug addict over an unpaid debt, and trying to dispose of his body using a wheelie bin.
Thomas Killen, 33, of North Lawn Court, and Brett Edwards, 37, of Okehampton Road, both denied murder but were found guilty at Exeter Crown Court.
They attacked and murdered James Woodhouse at a flat in Exeter where he'd been staying on 11 November 2016, having been invited to stay there by Edwards.
The court heard Mr Woodhouse was a former soldier who had become homeless after developing a major addiction to cocaine. He owed money to Killen and at least one other gang of drug dealers and there was said to be a bounty on his head which would be paid to anyone who could track him down and beat him up.
Six days after punching him until he was unconscious, Killen and Edwards returned to the flat with a stolen wheelie bin in an attempt to remove his body, the court heard.
Judge Sir John Royce will sentence them later.
