A GP surgery where four doctors have resigned at once has been placed in special measures.
The Beggarwood practice in Basingstoke "put patients at risk of harm", the Care Quality Commission (CQC) said.
CQC inspectors who visited in February said two of the GPs felt the practice was "clinically unsafe due to staff shortages".
The four GPs are leaving the surgery in June but operator Cedar Medical said it had "adequate staffing".
A man arrested in connection with a bomb hoax at a shopping centre in Oxfordshire has been released on bail.
The Orchard Centre, in Didcot, was evacuated following a report of a suspicious item at Sainsbury's.
The area remained cordoned off until lunchtime on Monday whilst police investigated the item.
Officers confirmed it was "not a danger to the public" and a 35-year-old man, from Didcot, was arrested on suspicion of making a bomb hoax.
The death of a 10-year-old boy who became tangled in a duvet cover which was being used as a curtain was an accident, a coroner has ruled.
Ashton Tuckey, who had Down's syndrome, was found by his mother who initially thought he was looking out of the window at their Isle of Wight home.
He died in hospital as a result of "compression to the neck" following the incident in Binstead, near Ryde, on 7 December 2014.

Thames Valley Police will feature in the Channel 4 documentary 'Catching a Killer'.
The film shows how the major crime unit solved Natalie Hemming's murder.
She was reported missing last year and found dead in woodland.
Her partner Paul Hemming was convicted of the murder and sentenced to life.
Jo Beverley, Natalie’s sister said: “If this documentary stops one more child losing their mother and one more family having to go through this nightmare then it has been worth every second."
Det Supt Simon Steel said: “The documentary is deeply moving and highlights the issues of domestic abuse. While tragically this became a murder investigation, it shows the efforts the force made to find Natalie for her family."
It will air tonight at 21:00 BST.
A teenager arrested over a "rogue" batch of ecstasy on the Isle of Wight has been released, police have said.
The 16-year-old boy from Shanklin had been held on suspicion of supplying class A drugs after two teenagers were hospitalised.
Hampshire Constabulary said the boy had been released from custody but remained under investigation.
A girl, aged 14, and a 15-year-old-boy were taken to hospital on Tuesday night but have since been released.

What's would be the perfect addition to a boiling hot day in half term week?
That's right, no water.
Residents of Burghfield and Mortimer have been without water for most of the day after an issue with a water pump nearby, say Thames Water.
I've been speaking to some residents who have come down to a playing field in Mortimer to pick up bottles of fresh water put on by the water company to tide them over.
There were a few grumbles, but mostly this was a perfect example of British stoicism. Thames Water's staff have been polite and are literally handing water out by the gallon.
The use of a local Facebook group has also been very helpful in informing people of the situation.

It is thought the red tablets in the "rogue batch" may contain ketamine or methamphetamine.

At about 23:00 on 18 May, the suspects entered Tesco Express on Silverdale Road, assaulted a member of staff and stole a quantity of cash.
They left on foot in the direction of Wokingham Road via Maiden Erlegh Drive and it is believed they might have been picked up by a vehicle.
The male member of staff was taken to hospital and has since been discharged.
Detective Constable Steven Martin said: "I'd also like to speak to two customers who were in the store just before the offence took place who may be witnesses. They are believed to be a man at 10.57pm who left in a car, and a woman at 10.59pm who left on her bike."
