Ruth Rankine, deputy chief inspector of general practice, said an inspection was carried out as a follow-up to an earlier CQC visit in May last year, when the surgery was rated "requires improvement".
She added: “It is worrying that despite the concerns and improvements identified at that first inspection, our team found a decline in the standards upon their return.
"Care plans were not always being entered into patients electronic records putting patients at risk of not receiving the right treatment and a lack of clarity over the safe monitoring of repeat prescriptions particularly for those using high risk drugs."
The practice will be re-inspected within six months to check whether improvements have been made.
Hundreds of police officers have attended the funeral of a PC who died after being hit by a stolen car.
PC Gareth Browning, 36, from Thames Valley Police, died on 1 April from injuries sustained in November 2013.
A police helicopter hovered in salute as the funeral cortege was led through a guard of honour at St Mary's Church in Chieveley, Berkshire.
His family followed in a private group behind, and his father was among those who paid tribute during the service.

It's looking like Southampton defender Virgil van Dijk will be a man in demand this summer.
Liverpool, Everton and Chelsea are all thought to be preparing bids for the former Celtic man, who hasn't played since January after picking up a pretty serious ankle injury.
Now I hear Van Dijk is on Manchester City's radar too.
There has been no bid for the 25-year-old yet - and Southampton executive director Les Reed is known for driving a hard bargain.
Van Dijk also has a long contract, to 2022, so whatever fee is eventually agreed, expect it to be a pretty big one.

Kimmeridge Coastguard has released more pictures of Tuesday's dramatic rescue of three people stuck on cliffs near Lulworth.
With cliff rescue teams unable to reach them, Portland's coastguard helicopter was called in to winch them to safety. All three were unhurt.
A new poll by Hampshire and Isle of Wight Wildlife Trust has found that people in Southampton don't feel they get out into the wild enough.
Of the 101 people polled, 92% feel that nature is important to them but 79% don’t think that they spend enough time out and about. Also, 89% think it’s important to help nature.
Deborah Tann, chief executive of the trust, said: "Whilst it’s great that so many people say it’s important to help nature, we know that less than half of Southampton’s city-dwellers currently do anything themselves and many would like to do more.
"It’s vital that people realise how easy it is – no matter where you live – to make a difference."

A 16-year-old boy has been arrested on suspicion of supplying a class A drug as part of an investigation into a "rogue batch" of ecstasy tablets on the Isle of Wight.
Hampshire Constabulary has also now released a picture of the pills in question.
Det Con Jessica Hughes said: "They pose a significant risk to people’s health and I would urge anyone who may have information about them to contact us immediately."
